Community Care Overview
Many clients would rather stay in their own home, and we do all we can to ensure their wishes are respected. An Aged Care Assessment Team (ACAT) assessment is required to qualify for a CACP and EACH package.
OneCare's Community Care Programs offer an extensive range of home and community based services for clients in the greater Launceston area in Northern Tasmania as well as the Kingborough area in the South.
For clients in the community who wish to remain in their own home and are eligible for community packages, we offer a range of services. Depending on need and circumstances, we can offer home based support with the following programs:
• Community Aged Care Packages (CACP)
• Extended Aged Care at Home (EACH)
• Veterans' Home Care
• DVA Nursing
• Full fee for service

Community Aged Care Packages (CACP)
The OneCare CACP offers flexible, in-home assistance to elderly clients who have been assessed as requiring low-level care and support.
Services, which can be tailored to meet individual needs, may include:
• Personal care
• Home help, including meal preparation
• Transport to appointments
• Entertainment program (weekly bus outing)
• 24 hour on-call assitance
• Case Management
• Shopping
• Gardening
• Respite care
• Nursing and allied health services as required.
Extended Aged Care at Home (EACH)
The OneCare EACH program is designed to assist frail aged people to remain in their homes, supported by personal care.
Services, which can be tailored to meet individual needs, may include:
• High-level care
• Home help, including meal preparation
• Transport to appointments
• Case management
• 24 hour on-call assistance
• Medication packaging
• Emergency in-home respite
• Entertainment program
• Consumables such as continence supplies and wound care products
• Shopping
• Gardening
• Respite care
• Equipment
• Nursing and allied health services as required.
Veterans' Home Care
OneCare's Veterans' Home Care services are designed to assist those veterans and war widows/widowers who wish to remain in their own home but need a small amount of practical assistance.
Services, which can be tailored to meet individual needs, may include:
• Personal care
• Domestic assistance
• Safety-related home and garden services
• In-home respite care
DVA Nursing
OneCare's DVA Nursing services deliver nursing care and support to veterans and war widows/widowers who wish to remain in their own home but need ongoing nursing care. To be eligible veterans/widows must be Gold Card holders, or approved White Card holders.
Services, which can be tailored to meet individual needs, may include:
• Support for the frail/aged to remain at home
• Comprehensive nursing assessment
• Follow up treatment after hospital discharge
• Wound care
• Continence care
• Dementia care
• Diabetes care
• Case management
• 24 hour on-call assistance
• Oxygen and respiratory care
• Palliative care
• Support for long term illness
• Other care provided to meet your needs (e.g. management of tracheostomy, colostomy, etc.)
